dentists can help you with all of your dental needs. And with our
dentists averaging fifteen years of experience, you know you are in
good hands. Our hygienists help promote your dental health with
routine teeth
cleanings and periodontal services including root planing and
scaling.
For our patients requiring more complex treatment, our network
includes dentists trained in all of the major dental specialties
including orthodontics, oral surgery, endodontics,
periodontics and pediatric dentistry.

For our patients without access to traditional dental insurance,
we offer the PERFECT TEETH™ Dental
Plan a reduced-fee dental plan available to individuals and
their dependents not currently protected by dental insurance or a
discount dental

New Patient Paperwork
If you are a new PERFECT TEETH™ patient and would like to
complete your new patient paperwork in advance of your appointment,
please click here.
The entire packet should be completed and brought with you to your
upcoming appointment. Please note, there may be additional
paperwork required upon check-in for your appointment.
Otherwise, please plan to arrive a minimum of 15 minutes early for
your appointment to complete the paperwork at the office.
